Instructions
Pick your moment
Choose the form that fits where you are in the year. Use the Reflect & Refine form for mid-year or post-initiative check-ins, the End of Year Reflection for final feedback, and the Start, Stop, Continue form anytime you want structured input on what to keep or change.
Distribute the forms
Hand them out at a staff meeting, leave a stack in the lounge with a return deadline, or walk them around individually. Let staff know their responses will be read and used.
Review and look for patterns
Read through responses and note what comes up repeatedly. Where is there strong agreement? What surprises you? Pull out two or three themes before your next staff touchpoint.

Post-initiative check-in
Run a quick round after a schedule change, new program launch, or other major shift. The Reflect & Refine form works well here with slight tweaks to the prompts.
Team-level reflection

Less is more
If you add your own questions, stick to three to five total. At your next meeting or in a quick email, tell staff what you learned and what you plan to do with it.
